AdventHealth Heart of Florida and AdventHealth Lake Wales open applications for 2026 community impact grant
| DAVENPORT, Fla. (Friday, March 05, 2026) -- AdventHealth Heart of Florida and AdventHealth Lake Wales have opened the application period for the 2026 Community Impact Grant, an initiative designed to expand community health programs and strengthen access to whole‑person care throughout Polk County. Local nonprofit and community-based organizations are invited to apply for one-year grant funding aimed at addressing some of the area’s most pressing health needs. The Community Impact Grant supports projects that deliver measurable improvements in community well‑being, with this year’s funding available in various amounts for organizations serving residents across Polk County. The program’s priorities were guided by findings from the 2025 Community Health Needs Assessments (CHNA) for AdventHealth Heart of Florida and AdventHealth Lake Wales, which identified mental health, health care access and quality and nutrition and healthy eating as the most significant needs facing the region. With these priorities in mind, the 2026 grant cycle will focus on supporting evidence-based, programmatic initiatives that enhance quality of life, reduce barriers to care and promote healthier communities. Eligible organizations should demonstrate their ability to deliver meaningful and sustainable outcomes, collaborate with community partners, and track progress through measurable metrics. Organizations applying for the 2026 Community Impact Grant must meet several eligibility requirements including:
